Form 10

The Annual Statement of Production (Form 10) is submitted to the Ohio Department of Natural Resources, Division of Oil & Gas Resources Management. One form needs submitted for each well that is capable of producing oil and gas. If you have more than 100 wells to report, then the information must be submitted electonically. You can both print the forms and create an electronic file to submit from within the software if you have purchased the reporting capability for the state of Ohio.

Refer to the ODNR website for deadlines and any additional information that is needed.

The steps that you will be taking within the software are described below:

Create Form 10 File:
Once all information that needs to be reported has been processed on a run closing, you can Create the Form 10 File. This will calculate the totals that need to be reported, and store the calculated totals in a different place for only this report. This ensures that if any changes are made to your core data after this report is filed you will still be able to see exactly what was filed because that information won't change.

Form 10 Summary:
After the totals are calculated, you can view this report to see what was calculated and is going to be reported to the state.

Modify Form 10 File:
This window displays the raw numbers that were calculated when you created the file. Each total can be edited directly on this window. Any changes made to this window will only affect the totals that are reported to the state. These changes may be needed for a correction, or in siituations where everything wasn't processed within SherWare because you started using the software mid-way through the period that is being reported.

Print Form 10 Report:
Prints the form and totals on blank paper that can be sent to the state.

Form 10 Magnetic File:
If you have more than 100 wells, or if you just want to file the information electronically, then you can use this to create a digital file that can be submitted to the state.
